Background
Despite Cardiac Rehabilitation's (CR) proven benefits, it is well-established that disparities exist within these programmes, with certain groups exhibiting lower participation rates (Matthews & Brewer, 2021). Identifying these patient groups and exploring factors influencing their engagement is crucial for developing targeted interventions to increase uptake and reduce local health inequalities.Aim
To understand which patient groups decline Core CR and which groups require further engagement within Leicestershire.Methods
Statistical analysis was performed on data for patients referred to University Hospitals of Leicester (UHL) CR in 2022 (n = 2158). Demographics and characteristics were compared between those who declined or accepted CR at initial contact. Patient characteristics were age, gender, ethnicity, language, marital status, initiating event and presence of co-morbidities.Results
1909 patients' data were analysed with 249 excluded due to being incomplete. 609 patients declined CR at first contact. Mean [SD] age of decliners was 70.5 years [12.9] which was significantly older than those who accepted CR (65.9 [11.8]) (p< 0.01) (table 1). The proportion of females (p=0.04), non-English speakers (p< 0.01) and those divorced/widowed/separated (p<0.01) declining CR at initial contact was significantly greater than the accept group. Acceptance rate was higher in co-morbid patients (p<0.01). More patients accepted CR following valve disease, Heart Failure, STEMI and Arrhythmia compared to those who declined. A higher proportion of patients declined CR following NSTEMI. There were no differences in acceptance of CR by ethnicity (p =0.31).Conclusion
CR is more likely declined by patients who are female, unmarried, non-English speaking or in older age groups. Patients were also more likely to decline CR following NSTEMI compared to other diagnoses. There was, however, no impact on uptake by ethnicity alone. These results inform of areas for CR at UHL to focus on for patient engagement and education in order to reduce inequalities or disparities.Год издания: 2023
