Esperança materna primária: histórias de mães de UTI Neonatal no contexto da prematuridadedissertation
Аннотация: Technological advances in medicine have allowed premature babies to survive outside the womb. However, the medical equipment and procedures used to support neonatal life end up restricting the care offered by the mother to her baby, an important aspect for the psychic development of the newborn.Regarding the mother-baby dyad, the pediatrician and psychoanalyst Winnicott establishes that the mother develops an identification with the child, through which she perceives and anticipates his needs. In the neonatal intensive care environment, the mother, who is in postpartum maternal state, faces the unique challenge of dealing with the fragility of her premature baby.Such context requires the mother to access her internal resources from previous environmental care to believe that she and her baby will be able to overcome these adverse conditions, trusting in their life potential. Thus, the present study aimed to investigate conditions related to hope of mothers of preterm infants through reports from a Neonatal ICU journal. Therefore, a content analysis was performed, and four categories were identified: How a Neonatal ICU mother is born, the adverse context in which hope is born; The support received, the human encounter in sustaining hope; Faith, the hope present in religiosity/spirituality; Positive perspective, the mark of hope. Thus, it was found that mothers of premature babies, who may present primary maternal concern, faced an intense situation of rupture of this primordial relationship, however, with the broad support of people in the hospital environment and with the belief in a divine intervention, they were able to dream about a future of restoring the bond with their child after hospital discharge, when they could fully experience motherhood.Such perspective allows essential conditions for health: a hopeful outlook on how to deal with mishaps of this life-threatening experience; integration of these experiences over the time; and maintenance of identification with the baby during hospitalization, which means considering herself a "NICU Mom".
