Аннотация:Backgroud: We tested the hypothesis that preterm infants who develop chronic lung disease (CLD) among those without preceding respiratory distress syndrome (RDS) have higher concentrations of matrix metalloproteinase-8 (MMP-8) in the tracheobronchial aspirates (TA) within 24 hours after birth than those who do not.Methods: TA MMP-8 concentrations were measured by enzyme-linked immunosorbent assay in the 27 preterm infants born before 34 weeks' gestation without preceding RDS.Multiple logistic regression analysis was done.Results: Median concentration of TA MMP-8 was higher in CLD group than in no CLD group [44.42 ng/mL (0.023-255.944)versus 3.47 ng/mL (0.960-40.734),Pϭ0.008].This difference persisted significantly after adjustments for the effects of gestational age at birth, 5 minute Apgar score less than 7, and histologic chorioamnionitis [Pϭ0.036;Odds ratio, 30.020; 95% confidence interval, 1.257-716.685].The diagnostic indices of MMP-8 (cutoff, 7.94 ng/mL) as a predictor of development of CLD in no RDS group were: sensitivity of 82.4%, specificity of 80.0%, positive predictive value of 87.5%, negative predictive value of 72.7% (Pϭ0.003).Conclusions: Increased TA MMP-8 concentration within 24 hours after birth may be a significant predictor of later development of CLD in neonates without preceding RDS.
