Аннотация: We agree with Keskinbora's comments that the mini-nuc technique1 is a cost-effective alternative to phacoemulsification. We have also found it to be a useful stepping stone in the learning curve of phacoemulsification. It enables the trainee to grasp the fundamentals of a self-sealing incision, a closed surgical system, and capsulorhexis without necessitating the use of the phacoemulsification machine. We have not found its learning curve longer than that of phacoemulsification. Doubts are often expressed about the effect of the mini-nuc technique on the cornea. We studied 64 eyes having this surgery. All eyes had a slitlamp examination and corneal endothelial cell count and morphology study preoperatively. Eyes with corneal pathology or an endothelial cell count of less than 1000 cells/mm2 were excluded. The eyes had uneventful cataract extraction by the mini-nuc technique and implantation of a poly(methyl methacrylate) posterior chamber intraocular lens. Postoperatively, 5 cases (7.8%) had transient corneal edema, which cleared by the seventh postoperative day. The mean endothelial cell loss at 6 months was 9.21% ± 2.49% (SD). This is not different from that reported after phacoemulsification.2 Fears regarding the effect of the mini-nuc technique on the cornea are unfounded. The use of an anterior chamber maintainer facilitates surgery that is safe for the cornea even without the use of ophthalmic viscosurgical devices.3 This should make the technique more attractive to surgeons in developing nations.
