Clinical Evaluation of the Revised Staging System for Acute Pancreatitis in Japanстатья из журнала
Аннотация: In 2008, the Japanese clinical staging system for acute pancreatitis was revised.1 This new staging employs 9 prognostic factors and an enhanced CT grading system that enables a wide variety of clinicians to easily make an assessment regarding the severity of acute pancreatitis. Using this new staging system, we will divide all pancreatitis patients into 2 categories, severe and mild. The aim of this study was to clarify the difference of severity for acute pancreatitis between the new staging system and the existing staging system in Japan. We evaluated this new staging system by comparing it to 217 previously evaluated pancreatitis cases. According to the new system, 37 (17.1%) cases were evaluated as severe and 180 (82.9%) cases were evaluated as mild. The rate of severe pancreatitis changed from 26.7% to 17.1%, and the rate of mild changed from 52.1% to 82.9%. We tried to make a conversion formula for the severity score between the existing system to that of the new system. The formula is < the new system's severity score = 0.418 + 0.332 × the existing system's severity score >. In light of this formula, 3 points from the new system would equate to 7.8 points in the existing system. The rate of mortality over 3 points in the new system's score is 14.3%. From this analysis, we can show that the new staging system's rating of severity is higher than the existing system. We have to enhance the recognition oft the new Japanese staging system's severity.2 When patients are diagnosed with 'severe pancreatitis' using this new system, they should be transferred to an intensive care unit where systemic management of severe acute pancreatitis is available.FIGURE 1: The conversion formula for severity score of the existing system to that of the newly revised Japanese system and the graphic representation of that formula.
