Periodic Spasms: An Unclassified Type Of Epileptic Seizure In Childhoodстатья из журнала
Аннотация: SUMMARY Seven boys aged between 14 months and 13 years with focal or multifocal partial epilepsy had very unusual seizures, characterized by series of periodic bilateral spasms. These consisted of a brief, complex movement of slight to severe intensity, mainly involving the axial muscles, with raising of the shoulders, adversion of the eyes and head, often with rictus‐type facial grimaces and occasional stretching of the legs and emission of a guttural sound. The ictal EEG showed a pattern of periodic complexes, characterized by a slow wave with superimposed fast activity. This form of epileptic seizure is resistant to treatment and is found only in patients affected by a brain disease consisting of a fixed or progressive encephalopathy. These observations document the existence of a rare form of seizure or epileptic event, not yet included in the International Classification of epileptic seizures and clinically relevant because of its diagnostic and prognostic implications. RÉSUMÉ Spasmes périodiques: un type non classé de crises épileptiques de l'enfance Sept garçons âgés de 14 mois à 13 ans présentaient une épilepsie partielle unie ou multifocale d'un type tres inhabituel, caractérisée par des séries de spasmes bilatéraux periodiques. lls consistaient en un mouvement bref, complexe, d'intensité modérée ou forte, impliquant principalement les muscles axiaux, avec haussement des épaules, deviation de la tête et des yeux, souvent des grimaces faciales à type de rictus, un étirement occasionnel des jambes et l'émission de sons gutturaux. L'EEG sous crise révélait des tracés de complexes périodiques, caractérises par une onde avec une activite rapide surimposée. Cette forme de crise comitiale résiste au traitement et ne s'observe que chez les sujets présentant une affection cérébrale à type d'encéphalopathie fixée ou évolutive. Ces observations sont en faveur de l'existence d'une forme rare de crise ou manifestation comitiale, non encore incluse dans la Classification Internationale des crises épileptiques et cliniquement significative en raison de ses implications diagnostiques et pronostiques. ZUSAMMENFASSUNG Periodische Spasmen: ein unklassifizierter Typ epileptischer Anfälle im Kindesalter Sieben Jungen im Alter zwischen 14 Monaten und 13 Jahren mit einer fokalen oder multifokalen partiellen Epilepsie hatten sehr ungewöhnliche Anfälle, die durch Serien von periodischen bilateralen Spasmen charakterisiert waren. Diese bestanden in einer kurzen, komplexen, mehr oder weniger ausgeprägten Bewegung, die hauptsächlich die axialen Muskeln betraf, mit einem Heben der Schuitern, einer Aversion von Augen und Kopf, oft mit grinsenden Grimassen, gelegentlichem Strecken der Beine und dem Ausstoßen gutturaler Laute. Das Krampf‐EEG zeigte periodische Komplexe, charakterisiert durch eine langsame Welle, die von schneller Aktivität überlagert wird. Diese Form der Krampfanfälle ist therapieresistent und wird nur bei Patienten mit einer Hirnerkrankung im Sinne einer stationären oder progressiven Encephalopathie gefunden. Diese Beobachtungen dokumentieren das Vorkommen einer seltenen Form von Krampfanfällen oder eines epileptischen Geschehens, die noch nicht in die Internationale Klassifikation epileptischer Anfälle aufgenommen sind, und die klinisch relevant sind wegen ihrer diagnostischen und prognostischen Folgen. Estos ataques consistian en un movimiento breve y complejo de intensidad ligera o grave, que afectaba mayormente los musculos axiales, con elevación de los hombros, aversión de ojos y cabeza, a menudo meucas faciales a modo de rictus y ocasionalmente extensión de las piernas y emisión de un ruido gutural. El EEG ictal mostró un trazado de complejos periódicos caracterizados por una onda lenta sobreimpuesta a una actividad rápida. Este tipo de ataque epiléptico es resistente al tratamiento y se halla sólo en pacientes con una alteración cerebral consistente en una encefalopatia estable o progresiva. Estas observaciones documentan la existencia de una forma rara de ataque o crisis epiléptica aun no incluida en la Clasificación Internacional de las Crisis Epilepticas y de importancia clinica por sus implicaciones diagnósticas i pronósticas.
